چکیده

Received 20 August 2014; revision accepted for publication 25 November 2014. We describe the case of a 17-year-old girl with a recent diagnosis of hypertrophic cardiomyopathy. Her main symptom was post-exercise dizziness without loss of consciousness. Two-dimensional transthoracic echocardiography revealed marked hypertrophy of the basal segment of the interventricular septum (27 mm) (figure 1, panel A) with no subaortic gradient at rest and during a Valsalva manoeuvre performed in the supine position (figure 1, panel B). The additional investigations did not reveal sustained or non-sustained ventricular tachycardia on 48-hour Holter monitoring nor hypotension during bicycle exercise testing. The patient underwent exercise echocardiography in a semi-supine position on a tilting exercise table. Echocardiographic imaging was continuous during exercise and recovery and did not reveal left ventricular outflow tract obstruction (LVOTO) (figure 1, Hypertrophic cardiomyopathy: obstruction fl ushed out with exercise echocardiography

Exercise eco-Doppler in hypertrophic cardiomyopathy patients. Determinant factors of exercise intolerance.

INTRODUCTION AND OBJECTIVES At-rest echocardiography is a poor predictor of exercise capacity in patients with hypertrophic cardiomyopathy. We aimed to test the performance of treadmill exercise Doppler echocardiography in the prediction of functional limitations in these patients. METHODS Eighty-seven consecutive patients with hypertrophic cardiomyopathy underwent treadmill exercise echocard...

What Is Really a Nonobstructive Hypertrophic Cardiomyopathy? The Importance of Orthostatic Factor in Exercise Echocardiography

The authors report the case of a 23-year-old girl with nonobstructive hypertrophic cardiomyopathy evaluated by resting echocardiography. The patient complained of syncope after playing basketball. The patient was submitted to treadmill exercise echocardiogram, and she exercised for 9 minutes in standard Bruce protocol. The left ventricular outflow gradient did not occur at peak workload; howeve...

Predominant, severe right ventricular outflow tract obstruction in hypertrophic cardiomyopathy.

Recently, new cardiac imaging techniques as cardiac magnetic resonance have shown to play a primary role for a recognition of unusual mechanisms of right and left ventricular obstruction. However, echocardiography remains the most widely used technique and the first step to recognize myocardial hypertrophy, blood flow velocity, and pressure gradients in real time (2). We describe a rare case of...

Delayed untwisting: the mechanistic link between dynamic obstruction and exercise tolerance in patients with hypertrophic obstructive cardiomyopathy.

OBJECTIVES We sought to determine the mechanisms linking dynamic obstruction and exercise tolerance in patients with hypertrophic obstructive cardiomyopathy (HOCM). BACKGROUND Patients with HOCM have reduced exercise tolerance. However, the association between dynamic obstruction and peak oxygen consumption (VO2max) is weak. METHODS We enrolled 25 patients with HOCM, 20 with hypertrophic ca...

Flail Subaortic Membrane Mimicking Left Ventricular Outflow Tract Obstruction in Hypertrophic Cardiomyopathy

A subaortic membrane is an uncommon cause for left ventricular outflow tract obstruction. Hypertrophic cardiomyopathy with dynamic left ventricular outflow tract obstruction would mask the presence of the subaortic membrane on transthoracic echocardiography and cause a false diagnosis. We report a patient with subaortic stenosis due to flail subaortic membrane misdiagnosed as obstructive hypert...
